Front brakes: when applied there is a jerky motion like they are catching and releasing.

when to replace brakes
Do you know why,when I am on the highway my steering wheel starts to shake when my speed gets up to 70?
And also when I am on the highway & getting off my exit I step on the brakes & the whole front end shakes, however it does not do it all the time, do you know what could be wrong?
